Item Description

This form can be used by people who want to volunteer to help people affected by the virus. 

The purpose of this form is to collect contact details so we can match volunteers with the tasks and services that are needed.

The form will ask

  • whether a volunteer is DBS checked.
  • if the volunteer can drive
  • whether they can make welfare phone calls.
  • which towns and villages they can support.

This form can be imported into Continuum XFP and edited and amended to meet your needs.
