Mapping Checkbox and Radio Button Fields

Mapping Checkbox and Radio Buttons require PDFs to be setup a certain way. You will need to already have, or will need to create a fillable PDF that can be used in conjunction with XFP. See Creating a suitable PDF for more further information.

Checkbox questions allow a user to ‘check all that apply’ where a Radio Button question only allows a user to select one answer.

Option Value or Label

In the examples below we use the checkbox and radio button option label but you can use the option value as well.

You'll need to be consistent and only use one or the other.

When creating you fillable PDF in Adobe Acrobat Pro if you set the checkbox "Export Value" or Radio Button Choice to the Label you must use the question label when mapping the template. Or vice versa if you use the question value in the PDF setup.

Checkboxes

  1. Create your form page with a checkbox question on the page.

    In this example we have a checkbox question with the question text: Please select all that apply.

    Then, we created Label values of:

    Option 1

    Option 2

    Option 3

    Option 4

  2. Create the checkbox question in the PDF using Adobe Acrobat Pro.

    The questions on this PDF must be using Adobe’s checkbox component

    You must ensure that the Name label and the Option - Export Value label match exactly to each of your checkbox label values in the XFP form.

    In Adobe Acrobat Pro:

  3. Map the form field to the PDF field in the Create PDF of Submission Template.

    When you map the form fields to the PDF fields, you will see separate PDF fields for each of the options on your checkbox field. You will just need to map each of those options back to the same checkbox question on your form

    This will now ensure that when the form is submitted, the checkboxes on the PDF will be populated accordingly.

Radio Buttons

  1. Create your form page with a radio button question on the page.

    In this example we have a radio button question with the question text: Please select the one that applies.

    Then, we created Label values of:

    Choice 1

    Choice 2

    Choice 3

    Choice 4

  2. Create the radio button question in the PDF using Adobe Acrobat Pro.

    The questions on this PDF must be using Adobe’s radio button component

    To map to a Radio Button field on a PDF you must ensure that the Name label on the PDF tool matches the question text from the Jadu form:

    You must also ensure that the Option - Radio Button Choice matches the label from the form question:

  1. Map the form field to the PDF field in the Create PDF of Submission Template.

    When you map the form fields to the PDF fields, you will see a single PDF field for the radio button with the available Choice Values listed. You will need to map this to the radio button question from your form.
