Continuum XFP #98

Bug fixes.

This release:

Fix Fixes an issue where, when creating a new page template for a form, clicking "Return to form" immediately after giving the page a title sometimes caused the form to break.

Fix Fixes an issue where lines in a CSV file output would not have the correct new line character.

Fix Fixes an issue where, when the PDF generation action was enabled, answers to checkbox questions were not populated into the PDF.

Compatibility

This release contains both Continuum #98 (5.11.1) and the corresponding Continuum CMS 18.7.0 in a single package. There is no need to apply the Continuum CMS package separately.
