The attached forms cover investigation into an outbreak, appropriate infection prevention and control management, supporting testing and contact tracing. 

This process will contribute to the delivery of the objectives of preserving lives, supporting the vulnerable and protecting livelihoods and reinforces the importance of a co-ordinated approach across multiple agencies to achieve this on behalf of your residents.

These forms and workflows can be imported into Continuum XFP and Jadu CXM and edited and amended to meet your needs.
