Bug fixes.

This release:

Fix Fixes an issue where a user could not interact with modals in the document editor

Modals are used to insert images, tables and links in the document editor, an issue with the positioning of the modals when setting up form templates for 'Emails' and 'Show content on form completion' resulted in the user being unable to interact with the modals.

Compatibility

This release contains both Continuum #95 (5.9.1) and the corresponding Continuum CMS 18.6.4 in a single package. There is no need to apply the Continuum CMS package separately.
